You may park directly in front of the Sage Farmhouse or use the lot that is up the hill.

Follow Ups - $70 (30 min)
HSA & FSA approved. Cash, Credit Card, Check and Paypal/Venmo are all accepted.
Payment is expected at day of service.

I am an out of network provider; upon request an itemized bill often referred to as a superbill can be provided to you after your session which can be submitted to your insurance for a reimbursement review.

However, if you have been managing your care with another physician and have relevant records please upload to your new patient intake or bring them along to your first visit.

No and yes. My office is located in a historic building that has not been ADA retrofitted and my office is on the 2nd floor. Please note that by request arrangements can be made to treat patients on the first floor of the building which has no steps to enter.
